Work Environment

Your working hours will be from 8:00 a.m. to 5:00 p.m., Monday through Friday, totaling 40 hours of dedicated service each week. As part of a collaborative team, you will work alongside physicians, nurses, case managers, and therapists to ensure our patients receive well-rounded care.

Key Responsibilities

• Conduct thorough assessments and effectively diagnose acute and chronic medical conditions, utilizing advanced clinical skills and patient history.
• Develop tailored treatment plans that incorporate patient preferences, lifestyle, and medical history, ensuring individualized care strategies.
• Prescribe appropriate medications and therapies, and order necessary diagnostic tests, while monitoring patient responses to treatment.
• Educate patients and their families on disease prevention, health maintenance, and lifestyle modifications to enhance overall health outcomes.
• Collaborate with interdisciplinary teams, facilitating seamless coordination of care and timely referrals to specialists when needed.
• Maintain accurate, up-to-date electronic health records (EHR) to ensure comprehensive documentation and compliance with regulatory standards.
• Participate in quality improvement initiatives, contributing to the development of best practices in patient care and adherence to clinical guidelines.
• Stay current with the latest advancements in healthcare practices and regulatory requirements to continually enhance your clinical expertise.

Patient Population / Client Focus

In this role, you will primarily serve adults residing in our Assisted Living Facility, many of whom may be experiencing chronic illnesses, mobility challenges, or mental health issues. You will have the opportunity to build meaningful relationships with your patients, understanding their unique needs and preferences, and tailoring your approach to fit their individual situations. Your work will directly contribute to the improved health outcomes and quality of life for these individuals.

Qualifications

• Current certification as a Nurse Practitioner from a recognized accrediting body.
• An active nursing license in the state of Texas.
• A Bachelor’s degree in nursing, with preference for candidates possessing a Master’s degree in nursing.
• A minimum of 2 years of clinical experience in a primary care setting, showcasing your ability to manage diverse patient needs effectively.
• Exceptional communication skills to facilitate effective patient engagement and education.
• Strong analytical and problem-solving abilities to address clinical challenges with precision.

Preferred Experience

While the primary requirement is a minimum of 2 years experience in primary care, candidates with experience specifically in geriatrics or working within assisted living facilities will be highly regarded. Compensation and Benefits

• Attractive salary range of USD 120,000 – 220,000 annually, depending on your experience and qualifications.
