Key Responsibilities

• Conduct thorough and comprehensive assessments to diagnose acute and chronic medical conditions, making swift clinical decisions tailored to each patient.
• Develop, implement, and manage evidence-based treatment plans in partnership with physicians and a deep network of healthcare professionals, ensuring a holistic approach to patient care.
• Order and interpret diagnostic tests, utilizing advanced medical technology to deliver accurate test results that inform patient management strategies.
• Prescribe and manage medications and therapies, monitoring their effective application and adjusting treatment plans as necessary.
• Educate patients and families on disease management, medication adherence, and comprehensive post-discharge care, empowering them to take charge of their health.
• Record and maintain detailed electronic medical records (EMR) that comply with healthcare regulations while ensuring clear communication across your multidisciplinary team.
• Actively participate in various quality improvement initiatives and clinical committees, contributing to the advancement of best practices and protocols in our organization.
• Engage in ongoing professional development and stay abreast of the latest healthcare trends, medications, and treatment innovations.

Qualifications

• Valid and current Nurse Practitioner certification with an active state nursing license.
• Bachelor’s Degree in Nursing from an accredited institution.
• Minimum two years of clinical experience as a Nurse Practitioner, with a focus on primary care or family medicine.
• Demonstrated proficiency in electronic health record (EHR) systems and familiarity with relevant medical software.

Compensation and Benefits
