Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our 51st Ave. Recovery Health Center. Full-Time Employed: 40 hours/week M-F 8:30a - 5:30p Location: 4616 N. 51st Ave. #108, Phoenix, AZ 85031 Additional Language Differential Pay Available NOW OFFERING AN ADDITIONAL $1.50/hr SMI DIFFERENTIAL Full Benefits Package, including 401K Generous PTO/Sick Time (4+ weeks in year 1) No experience required with a bachelors degree in behavioral health - we will train you New grads welcome Duties Include: Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals Completes assessments, crisis plans and individualized service plans Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources and in place. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions Basic understanding of the DSM-V preferred Certified Case Manager strongly preferred Must pass background check, TB test and other pre-employment screening. Must pass a typing assessment with a speed of at least 25 wpm.
