Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments that are age and culturally appropriate, addressing mental health, substance abuse, and medical needs within the context of a primary care setting.

• Deliver behavioral health interventions tailored to individuals, couples, and families. This includes intake assessments, ongoing counseling, treatment planning, crisis intervention, and coordinating referrals for comprehensive care.

• Serve as a liaison to community resources and social service agencies, facilitating access to medication assistance programs and related support services.
\n
• Collaborate with Quality Improvement teams to develop best practices for data collection, enhancing service delivery and client outcomes.
\n
• Maintain accurate records of client evaluations, progress notes, and counseling sessions to fulfill regulatory and compliance standards.

Patient Population / Client Focus\n

You will primarily work with individuals in the early stages of recovery from substance abuse disorders, as well as those facing co-occurring mental health issues. Your role is crucial in fostering an environment of trust and openness where clients can explore their challenges and develop strategies for sustainable recovery.

Qualifications\n

• Active and relevant state license as a Licensed Clinical Social Worker (LCSW).

• A Master’s degree in Social Work, with a minimum of three (3) years of post-licensure clinical experience in a mental health or substance abuse setting.

• Strong understanding of counseling techniques and crisis intervention, with an ability to make independent judgments regarding direct service provision.

• Exceptional skills in group facilitation and the planning of process-oriented discussions.

• Experience working in a detox or rehabilitation setting, particularly with individuals experiencing substance use disorders.

Compensation and Benefits\n
