Responsibilities
- Conduct PT evaluations and establish/adjust individualized treatment plans for residents in LTC and home health settings.
- Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, gait, functional independence, and pain management.
- Administer modalities and therapeutic exercises consistent with the plan of care; monitor patient response and modify treatment as needed.
- Document assessments, treatments, progress notes, and discharge planning in the electronic health record in compliance with privacy and regulatory requirements.
- Collaborate with the interdisciplinary team to coordinate care, educate patients and caregivers, and contribute to discharge planning and home safety education.

Requirements
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Active Texas Physical Therapist license or eligibility to practice in Texas.
- Experience in physical therapy; home health or LTC experience preferred.
- Ability to work effectively in home health and LTC settings and to travel to patient locations.
- Strong communication, documentation, and patient education skills.
- Ability to pass background checks and credentialing as required for employment.
