Responsibilities
- Plan and conduct segments of outpatient physical therapy programs to restore and improve physical, social and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Evaluate patients, establish measurable treatment goals, and implement individualized plans of care.
- Administer evidence-based interventions including therapeutic exercise, manual therapy, and modality applications as appropriate.
- Monitor patient progress, modify treatment plans as needed, and document outcomes per policy.
- Educate patients and families on home exercise programs, safety, and strategies to support function outside therapy sessions.
- Collaborate with care teams to coordinate goals, discharge planning, and follow-up.

Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent.
- California Physical Therapy license (active licensure or eligibility as required).
- Outpatient physical therapy experience preferred.
- BLS/CPR certification preferred.
- Ability to work in a travel assignment and adapt to new clinical environments.
- Current clinical practice in an outpatient setting and commitment to patient-centered care.
