As a member of the Hospice Center, your work will primarily involve collaborating with healthcare professionals, including nurses, doctors, and therapists, to develop and implement patient care plans, all from the comfort of your home office.

Key Responsibilities\n

• Develop and implement comprehensive treatment plans that address the psychosocial needs of patients, ensuring collaboration with the healthcare team throughout the treatment process.

• Perform detailed assessments of patients’ emotional, social, and behavioral challenges, providing tailored interventions to support their unique circumstances.

• Maintain accurate and timely documentation of clinical and progress notes in accordance with departmental policies, ensuring compliance and continuity of care.

• Provide crisis intervention services to patients and their families, equipping them with coping strategies to manage emotional distress during challenging times.

• Engage in regular consultations with patients’ families and support systems to ensure effective communication regarding treatment goals and discharge planning.

Qualifications\n
