In this role, you will deliver evidence-based diabetes self-management education to adult patients in hospital and outpatient settings. Responsibilities include assessing learning needs, developing individualized care plans, and teaching medication, insulin, nutrition, and lifestyle management. You will collaborate with an interdisciplinary team to improve outcomes, reduce complications, and support care transitions.
