Care Manager - Acute Care Facility

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5x8-Hour (08:00 - 16:30). Weekend coverage required, typically every other weekend.

Job Description

We are seeking a Care Manager with a focus on acute medical care and discharge planning. The ideal candidate will have experience in care coordination and must be prepared to manage a case load of up to 25 patients. This role does not require floating between facilities but requires working weekends as needed.

Responsibilities

• Perform care coordination and discharge planning for patients in an acute care setting.
• Evaluate patient needs and develop plans of care based on CMS guidelines.
• Ensure compliance with HIPAA regulations and evidence-based guidelines.
• Engage with interdisciplinary teams to support patient care and management.

Required Experience / Certifications / Licensure

• 2 years of experience in acute medical care management.
• Valid CA state RN license required; pending licenses accepted if obtained by start date.
• Experience with Epic charting system preferred.
• Must have previous charge experience in an acute care setting.
