Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional, social, and financial needs of patients entering the detox program.

• Develop and implement individualized treatment plans in collaboration with interdisciplinary teams, fostering a holistic approach to patient care.

• Provide individual and group counseling sessions to patients, focusing on issues such as adjustment, grief, behavioral health concerns, and coping strategies.

• Facilitate crisis intervention when required, offering immediate emotional support and guiding patients and their families through distressing situations.

• Coordinate with community organizations and resources to arrange post-discharge support, ensuring a comprehensive continuity of care for patients.

• Maintain thorough and timely documentation of interactions and interventions in compliance with organizational policies and regulatory standards.

Patient Population / Client Focus\n

Qualifications\n
