This role operates in a remote setting, allowing our LCSW to engage with patients from the comfort of their own chosen environment while still being a critical part of our Hospice Center team. Here, you will collaborate with a group of experienced professionals that includes psychiatrists, nurses, and fellow social workers, ensuring that our patients receive a continuum of care tailored to their specific needs. The position includes a flexible schedule with rotating hours that may require evening and weekend shifts to provide optimal support to our patients.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients’ mental health status, strengths, and needs, ensuring a personalized approach to care.

• Employ evidence-based therapeutic methodologies, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to effectively address and manage patients' challenges.

• Work collaboratively with a multidisciplinary team to ensure coordinated treatment plans that encompass psychiatric evaluations, medical interventions, and ongoing care.

• Educate patients and their families about diagnoses, treatment options, and medication regimens, fostering a partnership in the patient’s treatment journey.

• Facilitate individual, group, and family therapy sessions, utilizing evidence-based modalities that promote healing and coping strategies.

• Develop and update patient treatment plans in conjunction with the treatment team, ensuring alignment with patient goals and preferences to promote active participation in their care.

• Assist patients in navigating the complexities of the mental health system and advocate for their rights and needs to ensure equitable access to resources.

• Facilitate therapeutic groups and psychoeducation sessions, as well as actively participate in discharge planning meetings to promote successful transitions and ongoing support.

Patient Population / Client Focus\n

In this role, you will primarily serve patients within our Hospice Center, many of whom may be dealing with acute or chronic mental health issues related to grief, end-of-life transitions, and other significant life changes. Understanding the unique psychological challenges faced by these individuals and their families is essential in providing appropriate support and interventions that respect their emotional and psychological needs.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Tennessee.
