Work Environment

This position is primarily onsite, providing home care services. Working in patients' homes allows you to establish strong, trusting relationships with clients while delivering therapy tailored to their individual needs. Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from injury, illness, or surgery within their homes.
• Design and implement individualized treatment plans using various techniques, including exercise programs, manual therapy, and assistive devices, to alleviate pain and enhance mobility.
• Monitor and document patient progress, adjusting therapeutic goals and treatment approaches as necessary to ensure optimal recovery.
• Provide education and training to patients, their families, and caregivers on exercises, assistive devices, and self-management strategies.
• Engage collaboratively with a multidisciplinary team to optimize patient outcomes and share information about shared patients.
• Continuously improve professional knowledge through workshops, networking, and professional development activities.

Patient Population / Client Focus

Our Home Care Physical Therapist will engage with a diverse patient population, including a wide range of ages and medical conditions. Qualifications

• A Doctorate in Physical Therapy (DPT) is mandatory for this role.
• A valid state license to practice physical therapy in New Jersey is required.

• Proficient computer skills to manage patient records and documentation accurately.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist is preferred, although recent graduates may be considered.
• Previous experience in home care or similar settings is highly valued.
