Work Environment

This role is primarily based in a home care setting, which allows for a unique and individualized approach to physical therapy. You will travel to various patient homes in Trenton and nearby areas, where you will conduct evaluations, implement treatment plans, and monitor progress in a comfortable and familiar environment for the patients. Key Responsibilities

• Conduct thorough initial evaluations of patients to assess their physical, functional, and rehabilitative needs.
• Develop and implement individualized treatment plans that are evidence-based and tailored to each patient's condition.
• Measure and document patient progress regularly, making necessary modifications to enhance treatment effectiveness.
• Educate patients and their families about therapeutic exercises, pain management, and techniques to facilitate recovery.
• Maintain compliance with all relevant patient care policies, patient rights, and legal regulations.
• Ensure a clean, safe, and hazard-free environment while adhering to universal precautions during patient interactions.
• Participate in continuous education and in-services to uphold professional standards and improve service quality.
• Collaborate with other healthcare professionals to provide a multidisciplinary approach to patient care.
• Engage in performance improvement initiatives to enhance patient outcomes and clinic efficiency.

Patient Population / Client Focus

As a Physical Therapist at Adage Physical Therapy Clinic, you will work with a diverse patient population, including individuals recovering from surgery, those managing chronic conditions, and geriatric patients. Your role will involve understanding each patient's unique challenges and crafting interventions that emphasize functional recovery, mobility improvement, and pain management.

Qualifications

• Active physical therapy license in the state of New Jersey.
• Bachelor's Degree in Physical Therapy or a related field.
• Current CPR/BLS certification.

Preferred Experience

• At least one year of clinical experience in physical therapy; however, recent graduates will also be considered based on their educational background and clinical placements.
• Knowledge of federal and state regulatory requirements surrounding physical therapy practice.

Compensation and Benefits
