Nurse Case Manager Overview

Remote care management role supporting members through assessment, planning, coordination, and advocacy. Conduct comprehensive member assessments and develop individualized care plans. Coordinate care with interdisciplinary teams, providers, and community resources. Provide disease management education and support to improve health outcomes. Perform telephonic outreach and member engagement activities. Document care management activities accurately and timely. Support Medicare and Medicaid populations through evidence-based care management practices. Ensure compliance with regulatory, accreditation, and organizational requirements. Work independently while collaborating virtually with internal teams. Maintain productivity, engagement, and quality metrics.

Job Duties

• Support team objectives and operational initiatives
• Meet productivity and quality performance standards
