Key Responsibilities\n

As a Licensed Professional Counselor at Yakima Family Health Services, you will:

• Conduct thorough patient intakes, performing diagnostic and functional evaluations to develop individualized treatment plans in collaboration with primary care providers.

• Provide brief, focused interventions for patients encountering mental health crises, facilitating immediate support and stabilization.
\n
• Advise healthcare providers on appropriate referrals to specialty mental health services, ensuring that patients receive the most suitable care for their needs.

• Initiate follow-up contacts to monitor patient progress and assess the effectiveness of treatment strategies, making adjustments as necessary.

• Develop relapse prevention plans and assist at-risk patients in maintaining stability and transitioning out of crisis.

• Document patient progress accurately within treatment charts and keep the care team informed of any significant developments in patient conditions.

• Engage with patients to provide self-management skills education and coping strategies, promoting independence in their treatment journeys.

• Consult with clinic management on program outcomes and behavioral health service improvements.

• Facilitate compliance with medical treatments prescribed by primary care providers, assisting patients with strategies to manage treatment challenges.

Qualifications\n

To qualify for this position, candidates must possess:

Preferred Experience\n

Compensation and Benefits\n

The Licensed Professional Counselor position offers a competitive salary range of $87,000 to $105,000 per year, commensurate with experience and qualifications.
