Work Environment

This position is onsite with a focus on home care, allowing you to engage with patients within the comfort of their own homes. Key Responsibilities

• Conduct thorough initial evaluations and regular assessments to create individualized treatment plans tailored to each patient's unique conditions and goals.
• Implement therapeutic interventions, including manual therapy, therapeutic exercises, and advanced treatment modalities, to promote healing and improve mobility.
• Continuously monitor patient progress, adjusting treatment plans as necessary to adapt to changes in patient needs and outcomes.
• Accurately document all patient care activities in our electronic medical record (EMR) system to ensure compliance with regulatory and ethical standards.
• Educate patients and their families on recovery techniques, self-management strategies, and injury prevention to empower them in their healing journeys.
• Participate in regular interdisciplinary team meetings to discuss patient progress and refine treatment strategies alongside physicians and other healthcare professionals.
• Maintain knowledge of the latest best practices in physical therapy and ensure that all services meet California's PT licensing and documentation standards.

Patient Population / Client Focus

The clientele served by Gulf Side Physical Therapy includes individuals recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. We work with patients of all ages and physical abilities, from post-operative care to rehabilitation after sports injuries, all within a home care setting that allows for a personalized approach to therapy.

Qualifications

• A current and valid Physical Therapist license in California or eligibility to obtain one.
• A Bachelor's Degree in Physical Therapy from an accredited institution; a Doctorate in Physical Therapy (DPT) is preferred.
• CPR and BLS Certification, ensuring readiness for emergency procedures.
• Exceptional communication and interpersonal skills to effectively interact with patients, families, and team members.
• Proficiency in using EMR systems for accurate documentation and patient management.
• A minimum of two years of clinical experience, with a preference for candidates having experience in outpatient, orthopedic, or sports rehabilitation settings.
• Bilingual candidates, especially those fluent in Spanish, Mandarin, or Tagalog, are highly encouraged to apply.
