Registered Nurse – Case Management in Saint Helena, California. Start date 09/21/2026 for a 13-week travel assignment. Weekly pay range $2,271–$2,429 per week. Guaranteed hours 40 per week. Responsibilities
- Plan, organize, and direct home care services to meet patient needs in the home, managing a caseload in accordance with the patient’s plan of care.
- Conduct initial and ongoing comprehensive nursing assessments and document findings per policy.
- Coordinate patient care with other disciplines, integrating input to meet care goals and ensure timely updates to the plan of care.
- Provide direct and/or indirect skilled nursing care, maintaining safe, high-quality practice and monitoring patient outcomes.
- Educate patients and family members about nursing care, rights, responsibilities, and preventative health measures; provide written materials as appropriate.
- Ensure accurate and legible documentation of patient care delivery, status, response to treatment, and therapeutic interventions; update medication profiles as changes occur.
- Promote adherence to legal and policy requirements and collaborate with the care team to achieve mutually agreed-upon goals.

Requirements
- Active RN license in the state of practice (California)
- Bachelor’s Degree in Nursing (BSN) preferred
- At least two years of recent experience in the field
- Current CPR or BLS certification
- EPIC experience required
- Valid driver's license and auto insurance; reliable automobile; must be 21 years of age or older
- Ability to work in a home-based care setting and coordinate with multidisciplinary teams
