Job Description

We are seeking skilled home care professionals with experience in home health care. Candidates must possess the ability to provide quality care to patients in a home environment, including assessments and medical care management.

Required Skills

• Skilled nursing experience in home care (private duty nursing experience alone is not acceptable)
• Training and experience with OASIS
• IV skills and wound care
• General Medical Diagnoses knowledge
• Experience with high-risk diagnoses such as Heart Failure (HF), Chronic Obstructive Pulmonary Disease (COPD), Myocardial Infarction (MI), Pneumonia, Stroke, and Post-operative care

Certifications Required

• BCLS Certification

Uniform Requirements

Solid color scrubs are required.

Unit Information

Daily responsibilities include:

• Cardiopulmonary assessments
• Basic wound care
• ABI compression dressing changes
• Wound vac dressing changes
• Pleurx/Aseptic drain care
• Foley catheter management
• Staple/suture removal
• Glucometer usage
• Pro-time machine utilization
• Subcutaneous (SQ) and Intramuscular (IM) injections
• Lab draws via venipuncture
• IV skills and care including PICC line or Midline care and dressing change
• SQ port access/deaccess

Home Care Details

• Home skilled visits: 6-8 per day
• Work hours may vary depending on the HomeCare office needs:
• • Monday - Friday: 8 AM to 5 PM
  • Monday - Friday: 9 AM to 6 PM
  • Thursday - Monday or Friday to Tuesday shifts
  • Alternating weekend shifts

Additional Requirements

• Minimum of 1 year of home care experience required
• Valid driver's license and reliable vehicle for home visits
• COVID-19 vaccination required or a valid exemption on file
• Flu vaccination required or valid declination, with mandatory mask use while at work
• Candidate must have recent hands-on patient care experience within the last three months

Facility Environment

• Patient age groups: Adults, Geriatrics
• Daily census ranges from 400 to 1000 across locations
• Candidate may float within hospital or to affiliate hospitals nearby
• Smoke-free facility
