About This Role The Case Manager Continuing Care Coordinator RN-Per Diem is responsible for coordinating patient care and managing health conditions for defined patient populations. This role is ideal for a registered nurse with leadership experience who excels in a multidisciplinary team environment. Highlights Pay: $78 - $95/hour Location: Downey, CA Schedule: Per Diem What You'll Do Plan, develop, assess, and evaluate care provided to members. Evaluate and develop baseline medical and psychosocial evaluations and individualized patient care/treatment plans. Recommend alternative levels of care and ensure compliance with federal, state, and local requirements. Develop individualized patient/family education plans focused on self-management. Encourage members to follow prescribed courses of care (e.g., drug therapy, physical therapy). Coordinate care/services with utilization and/or quality reviewers and monitor level and quality of care. Coordinate the interdisciplinary approach to providing continuity of care, including utilization management and discharge planning. Make referrals to appropriate community services and outside providers. Consult with internal and external physicians and healthcare providers regarding continued care/treatment. Develop and maintain case management policies and procedures.
