Work Environment

This position operates onsite within our vibrant rehabilitation center, where collaboration with a multi-disciplinary team of healthcare professionals is paramount. Key Responsibilities

• Conduct comprehensive assessments of new patients to evaluate their physical capabilities, develop personalized therapy goals, and establish treatment plans tailored to individual needs.
• Implement evidence-based therapeutic interventions including manual therapy, therapeutic exercises, balance training, and mobility-enhancing techniques to facilitate recovery.
• Educate patients and their families on safe mobility practices, home exercises, and post-discharge strategies to ensure continued progress and independence.
• Maintain meticulous documentation of patient progress, treatment plans, and outcomes in compliance with facility regulations and industry standards, using our electronic health record (EHR) system.
• Collaborate effectively with fellow healthcare professionals including nurses and caregivers to coordinate patient care and optimize therapeutic effectiveness.
• Engage in ongoing professional development and training opportunities to keep abreast of the latest practices and innovations in physical therapy.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program (DPT or MPT preferred).
• A current Physical Therapy License in the State of New York is mandatory.
• Certification in Basic Life Support (BLS) is required to ensure the safety of our patients.
• Strong understanding of the New York State Physical Therapy Practice Act and adherence to ethical standards set by the American Physical Therapy Association.

Preferred Experience

• At least two years of experience as a physical therapist, preferably in a skilled nursing or outpatient setting.
• Practical experience in conducting evaluations and developing treatment plans tailored to the needs of geriatric patients.
• Experience with outpatient rehabilitation and a hands-on approach to therapy is advantageous.

Compensation and Benefits
