Key Responsibilities\n

• Provide direct clinical social work services to patients experiencing severe emotional disorders or symptoms of major mental illness, either in individual or group settings.

• Develop and implement personalized integrated care plans that ensure access to necessary support services and facilitate smooth communication amongst clients, families, and healthcare providers.

• Conduct brief, evidence-based assessments to identify patient needs, followed by the arrangement of additional evaluations where necessary.

• Monitor client progress through regular follow-up meetings, ensuring a clear focus on treatment goals and adherence to discharge criteria.

• Participate in crisis intervention efforts, delivering timely support to clients experiencing acute distress.

• Collaborate as a team member, sharing insights and strategies to advance common goals and thereby enhance overall service delivery.

• Develop and conduct educational sessions focusing on prevention and treatment strategies for clients, families, and caregivers.

• Fulfill documentation and data submission requirements efficiently, ensuring all reports are completed in a timely manner to maintain compliance.

• Provide on-call coverage as needed to offer ongoing support to patients and team members.

Qualifications\n

Life Assure Healthcare, PLLC offers a competitive salary range of USD 70,000 – 85,000 annually based on experience.
