This fully remote position allows for flexibility in your work schedule while ensuring you meet the varied needs of our patients. Although your primary responsibilities will be work-from-home, you may occasionally engage with healthcare teams or community organizations in person. Your schedule will encompass day, evening, and weekend shifts as needed, with on-call responsibilities that may arise based on patient requirements and departmental needs.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify patients’ emotional, social, and financial needs, shaping personalized care strategies.

• Provide individual and group counseling sessions to patients dealing with adjustment, grief, and coping challenges, using evidence-based practices.

• Coordinate care with interdisciplinary teams to ensure comprehensive management of patients’ psychological well-being, enhancing treatment efficacy.

• Develop and implement discharge plans, connecting patients with community resources to ensure a smooth transition to post-discharge care.

• Facilitate crisis intervention, delivering immediate support to patients and their families in distressing situations.

• Maintain accurate and timely documentation in compliance with legal and hospital requirements, ensuring adherence to quality care standards.

• Actively participate in hospital committees, contributing your expertise to advance the quality of social work practices within our organization.

Qualifications\n
