Key Responsibilities\n

• Engage with clients experiencing severe emotional disorders and mental illnesses both in virtual sessions and through community outreach initiatives.

• Conduct thorough assessments and create personalized care plans that integrate various health services, ensuring a holistic approach to patient care.

• Monitor client progress through regular follow-ups and adjust treatment plans as needed, always aiming for optimal outcomes based on clients' individual desires and conditions.

• Offer crisis intervention services as necessary, addressing immediate needs and ensuring the safety of clients during critical moments.

• Maintain accurate and up-to-date documentation, including treatment reports, session notes, and data submissions, to comply with legal and organizational standards.

• Participate actively in team meetings and committee work aimed at improving service delivery and quality care, sharing insights and strategies that benefit both clients and the organization.

Patient Population / Client Focus\n

In this role, you will primarily support clients receiving hospice care. This patient population can include individuals facing terminal illnesses along with their families, requiring a high level of empathy, understanding, and individualized attention. Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution. A Master's degree and LCSW licensure will be required for this role.

Compensation and Benefits\n

This position offers an annual salary in the range of USD 70,000 – 85,000, commensurate with experience and qualifications.
