Work Environment\n

Our clinicians operate within a Hospice Center setting, allowing for a supportive and collaborative remote work environment where employees can engage with colleagues from various disciplines. This role requires a blend of telehealth services and in-person visits when necessary, providing flexibility while ensuring continuous support for our clients. You will work Monday to Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs.

Key Responsibilities\n

• Conduct extensive assessments to identify the psychosocial needs of senior clients, encompassing their emotional, mental, and social well-being.

• Collaborate closely with clients and their families to develop personalized care plans that reflect individual goals, preferences, and circumstances.

• Provide counseling and emotional support during critical life transitions, helping seniors navigate challenges such as grief, relocation, or health changes.

• Act as an advocate for clients, facilitating access to community resources and ensuring they receive the necessary support services.
\n
• Coordinate care by working alongside an interdisciplinary team, including nurses and healthcare professionals, to enhance treatment outcomes.

• Visit clients regularly to monitor their mental and emotional well-being, assess living conditions, and address any safety or concern issues.

• Facilitate effective communication among family members, helping to resolve conflicts related to caregiving and support.

• Educate clients and families about available community resources to improve social engagement and overall quality of life.

• Keep accurate records of client assessments, care plans, progress notes, and all interactions, ensuring compliance with ethical standards.

• Stay abreast of current social work practices, policies, and regulations.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

This full-time position offers an annual salary range of $70,000 – $100,000, commensurate with experience.
