Key Responsibilities\n

• Conduct comprehensive assessments to identify the emotional, mental, and social needs of senior clients.

• Collaborate closely with seniors and their families to develop customized care plans that honor their goals and preferences.

• Provide vital counseling and emotional support, guiding clients and families through significant life transitions and challenges.

• Advocate for clients' rights, ensuring they have access to essential community resources and support services.

• Work as part of a multidisciplinary team, coordinating care with nurses, caregivers, and other healthcare professionals to optimize client outcomes.

• Conduct regular check-ins via video conferencing to monitor clients’ well-being, and evaluate their living conditions, addressing any concerns that arise.

• Facilitate effective communication among family members involved in the care process to resolve conflicts and enhance collaborative support.

• Educate clients and families about community resources, support groups, and programs that can enrich their quality of life.

• Maintain meticulous records of client assessments, care plans, progress notes, and interactions to ensure continuity of care.

Qualifications\n

Compensation and Benefits\n

The salary for this full-time position ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
