Key Responsibilities\n

As a Licensed Clinical Social Worker, your key responsibilities will include:

• Engaging directly with clients to provide clinical services focused on mental health, including assessment, therapy (individual, group, and family), and crisis intervention.

• Creating and implementing integrated care plans that address the unique needs of patients, while facilitating connections with additional services and resources.
\n
• Conducting thorough, evidence-based screenings followed by more in-depth assessments as necessary, ensuring clients receive tailored interventions.
\n
• Monitoring client progress and outcomes, and adjusting treatment plans based on individual needs and requests.
\n
• Maintaining accurate and timely documentation regarding client interactions, assessments, and care plan implementations, in adherence to organizational and ethical standards.

• Participating in team meetings and initiatives aimed at quality improvement and the advancement of agency goals.

Patient Population / Client Focus\n

This position will center primarily on serving hospice patients who face major mental health challenges and serious emotional disorders associated with terminal illnesses. You will be tasked with providing not only direct psychological support but also advocating for the emotional and psychosocial well-being of patients and their families during what can be an incredibly difficult time.

Qualifications\n

To be successful in this role, candidates should meet the following qualifications:

• A Master's Degree in Social Work, with a valid LCSW license to practice in Texas.

• Proficient knowledge of mental health assessment, diagnosis, and evidence-based treatment approaches.
\n
• Strong communication skills, both verbal and written, to effectively engage with clients, families, and team members.

• Ability to demonstrate cultural competency and respect all individuals regardless of their background.

• Proficient organizational skills, with the capacity to manage a caseload effectively and responsibly.

Preferred Experience\n

While a minimum of 2 years’ experience as a licensed clinician is required, prior experience working in hospice or end-of-life care is highly preferred. Familiarity with trauma-informed care practices and an understanding of the specific needs within a hospice setting will be advantageous for candidates.

San Ysidro Wellness Center values the contribution of our employees, offering a competitive salary ranging from USD 70,000 to 85,000 per year.
