We are currently seeking a qualified Full-Time Physical Therapist to join our dynamic home health team. This position requires hands-on patient care in their residential settings, focusing on functional mobility, reducing pain, and preventing disability.

This is not a remote job. The chosen candidate will be expected to travel to various home locations to provide therapy and care tailored to individual patient needs. As a Physical Therapist in Home Health, you will play a crucial role in helping patients regain or enhance their daily living and physical capabilities.

Duties and Responsibilities

• Conduct assessments of patients’ physical conditions based on their medical history and physician referrals.
• Develop and implement individualized care plans that focus on the promotion of health and treatment of conditions.
• Provide direct patient care aimed at improving mobility and alleviating pain.
• Instruct patients and their families on proper home care strategies and exercise routines.
• Maintain accurate and detailed patient records, including conditions, treatments, and progress.
• Regularly evaluate and update patient care plans in collaboration with healthcare team members.
• Engage in patient and family education about lifestyle choices, proper body mechanics, and the use of assistive devices.
• Apply therapeutic treatments using physical agents and techniques which may include, but are not limited to, massage, stimulation, therapeutic exercises, and hydrotherapy.
• Travel to patients' homes to deliver physical therapy sessions and follow-up on their care.
• Coordinate with other healthcare professionals to maximize patient recovery and progress.
• Adhere to professional and ethical guidelines as outlined by the American Physical Therapy Association (APTA).

Requirements

• Must possess a valid Professional Physical Therapist License.
• Current certification in CPR (Cardiopulmonary Resuscitation).
• Mandatory possession of a valid Driver’s License and proof of auto insurance.
• Proven experience in physical therapy, preferably in a home health setting.
• Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and medical professionals.
• Ability to work independently and manage a flexible schedule.
• Physical capability to perform job functions, including the ability to move equipment and lift patients as needed.
• Strong organizational skills with attention to detail and the ability to multitask.
• Commitment to ongoing professional development and continuing education.
• Sound judgment and the ability to make decisions in emergencies.
• Understanding of and compliance with HIPAA guidelines and other federal and state regulations pertaining to patient care and privacy.
