Home Health Licensed Practical Nurse

Provide skilled nursing care to patients in their homes under the direction of an RN Implement individualized care plans with patients, families, and the healthcare team Educate patients and families on disease management, treatment options, and self-care techniques Maintain accurate documentation of patient visits, interventions, and outcomes Communicate promptly with patients, physicians, clinical managers, and healthcare team members regarding needs and status changes Participate in care coordination and discharge planning Report to the Clinical Manager Support patient recovery, rehabilitation, and independence in the home setting Requirements Current nursing license in the practicing state Valid driver's license Auto insurance Reliable transportation Current CPR certification Two years of experience as an LPN/LVN in a clinical setting Ability to complete required tuberculosis screening Ability to maintain personal vehicle liability insurance meeting state minimum limits or specified Humana limits Preferred: nursing experience in a Home Health or Hospice setting Core Competencies Demonstrates expertise in providing skilled nursing care in home settings, implementing individualized care plans, and educating patients and families on disease management and self-care techniques.
