Field Outreach Case Manager — Community Health Advocate

Rescue Mission

The Rescue Mission invites applications for the SOS CTI Care Manager (Outreach) in Syracuse. This full‑time role centers on street outreach, coordinating needs before and after moves to shelter or home, and advocating for members facing discrimination or healthcare inequities. You will work with substantial independence in the field, with input from a Team Lead as needed, and be guided by person‑centered care and harm‑reduction principles throughout daily tasks. #J-18808-Ljbffr Rescue Mission
