This full-time position is designed for remote work, enabling you to provide vital services while enjoying the flexibility of a home-based office. You will maintain regular business hours, with occasional evening and weekend hours required to support special events and programs. Your work will directly facilitate comprehensive social services through a hospice center framework, which specializes in delivering holistic care to terminally ill patients and their families.

Key Responsibilities\n

• Conduct thorough assessments to evaluate clients' social, emotional, and psychological needs; utilize assessment results to develop tailored service plans.

• Provide individual and group counseling using evidence-based therapeutic approaches to assist clients in coping with diverse challenges.

• Develop and implement actionable service plans that align with clients' identified goals and objectives to promote empowerment.

• Advocate for clients’ rights and facilitate access to essential resources while addressing prevalent social justice issues.

Patient Population / Client Focus\n

Your role primarily focuses on individuals and families utilizing hospice services. You will address their unique emotional and psychological needs during profoundly challenging times, facilitating supportive conversations and implementing care plans that prioritize dignity, respect, and meaningful connections. As you work with clients facing end-of-life transitions, your interventions will empower them to express their needs and desires, ensuring that they receive compassionate support during their journey.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active and unrestricted state licensure as a Licensed Independent Clinical Social Worker (LICSW) is mandatory.

Compensation and Benefits\n
