Work Environment

Our practice operates in an onsite setting in Huntington, New York, where Family Medicine Nurse Practitioners collaborate with a multidisciplinary team in a supportive and dynamic environment. You will experience a blend of clinical responsibilities and community engagement, which may include home visits for patients unable to access traditional clinical settings. The typical work schedule ranges from 9:00 a.m. to 6:00 p.m., with flexible evening clinic options until 8:00 p.m. and occasional weekend outreach events.

Key Responsibilities

• Conduct thorough health assessments to determine patients’ medical histories and current health concerns during in-person visits or home care scenarios.
• Diagnose and manage a wide range of acute and chronic illnesses, implementing evidence-based care strategies tailored to individual patient needs.
• Promote preventive healthcare practices through health education initiatives and workshops that empower patients and their families.
• Coordinate referrals, ensuring seamless continuity of care and effective follow-up for all patients.
• Document patient encounters accurately and maintain compliance with regulatory standards and quality improvement protocols.
• Participate in community outreach efforts including health fairs, providing screenings, education, and resources to underserved populations.
• Stay current with advancements in community health trends, best practices, and public health policies, contributing to continuous improvement initiatives.

Patient Population / Client Focus

As a Family Medicine Nurse Practitioner at Eastside Internal Medicine, you will work with a diverse array of patients, focusing on those from underserved communities. Your role will include not only clinical care but also advocacy for health equity and reducing barriers that prevent patients from accessing necessary services. You will address a variety of health conditions, ranging from preventive care to chronic disease management, emphasizing culturally sensitive practices that respect the backgrounds and needs of each individual.

Qualifications

• Current Nurse Practitioner certification and an active nursing license in the state of New York.
• A Master’s degree in nursing from an accredited institution.
• Demonstrated expertise in clinical practice with a strong foundation in family medicine.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings.
• Experience with concierge medicine or house call practices is highly advantageous.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 annually, commensurate with your experience and qualifications.
