This position will be onsite at our home care facility located in the heart of Huntington. Key Responsibilities

• Conduct comprehensive psychiatric assessments and detailed evaluations to accurately diagnose mental health conditions in patients.
• Manage medication regimens and monitor treatment effectiveness, adjusting therapies as necessary to ensure optimal patient outcomes.
• Collaborate effectively with primary care providers, social workers, and case managers to create and implement integrated care plans.
• Provide immediate crisis intervention and emotional support, utilizing trauma-informed practices to stabilize patients in distress.
• Engage in community outreach, delivering educational initiatives that foster mental health awareness and reduce stigma.
• Offer therapeutic support through professional counseling sessions and psychoeducational workshops that promote self-care and coping strategies.
• Maintain comprehensive and confidential patient records, ensuring compliance with HIPAA and organizational policies.

As a Psychiatric Nurse Practitioner at Vitality Health Clinic, you will engage with a diverse patient population, including individuals from various cultural and socio-economic backgrounds. Qualifications

• A Master’s Degree in Nursing with a specialization as a Psychiatric Nurse Practitioner is required.
• State licensure as an Advanced Practice Registered Nurse (APRN) with psychiatric credentialing.
• Ability to demonstrate strong analytical skills and a deep understanding of psychiatric diagnoses and treatment modalities.
• Exceptional communication and interpersonal abilities for effective collaboration with patients and team members.

Preferred Experience

• Previous experience in a community health center or similar integrated care environment is highly favorable.

Compensation and Benefits

We offer a competitive salary range of USD 200,000 – 250,000 annually, commensurate with qualifications and experience. Our comprehensive benefits package includes:
