Key Responsibilities

• Conduct thorough physical examinations for patients, assessing their health status and necessary interventions.
• Diagnose and effectively manage both acute and chronic health conditions, ensuring a holistic approach to patient care.
• Order, perform, and accurately interpret various diagnostic tests including but not limited to medical imaging and laboratory tests.
• Develop individualized treatment plans in collaboration with the healthcare team, ensuring optimal medical care and resource utilization.
• Administer medications and prescribed treatments while monitoring patient reactions and progress.
• Maintain meticulous medical records using ICD-9 coding to support accurate healthcare documentation and billing processes.
• Engage in interdisciplinary collaboration with physicians, nursing staff, and allied health professionals to coordinate patient care strategies.
• Provide education and support to patients and their families on disease prevention, health maintenance, and treatment adherence.

Patient Population / Client Focus

You will primarily serve residents of an Assisted Living Facility, which includes a diverse population ranging from seniors requiring ongoing health management to those seeking urgent medical attention. Your role will involve addressing common health issues and chronic diseases that require immediate attention, as well as working proactively to prevent complications through comprehensive care and education.

Qualifications

• A valid Nurse Practitioner license and certification to practice in New York State.
• A minimum of a Bachelor’s Degree in Nursing from an accredited institution.

Preferred Experience

• At least two years of clinical experience in cardiology or urgent care settings, equipped to handle complex patient scenarios.
• Previous work experience in assisted living or memory care environments is highly desirable.
• Proficiency with electronic health records (EHR) systems, particularly Cerner, is a plus.
