Work Environment

As a Nurse Practitioner at our clinic, you will be part of a collaborative care team that includes physicians, physician assistants, nurse practitioners, medical assistants, as well as dedicated support staff like medical scribes, registered nurses, and care managers. The clinic operates in a traditional hospital setting, which allows seamless access to acute care services and supports a holistic approach to patient management. Key Responsibilities

• Provide exceptional primary care services to a diverse elderly population, conducting thorough assessments and managing a wide range of health concerns.
• Perform routine and acute office visits, allowing for timely diagnoses and appropriate interventions.
• Conduct annual wellness visits and health risk assessments, paying close attention to documentation that reflects patients’ comprehensive health needs.
• Coordinate patient care by liaising with other healthcare providers, specialists, testing facilities, and community agencies to facilitate high-quality patient outcomes.
• Lead population health initiatives, ensuring that screening protocols (e.g., breast cancer screenings) are met and adhered to.
• Assist the care team with phone triage and patient outreach to enhance care continuity.
• Educate patients on their health conditions and treatment plans, aiming to empower them in their healthcare journeys.
• Engage in community outreach and promotional activities to expand the clinic's services and improve public health awareness.
• Conduct home visits for patients with mobility challenges, ensuring comprehensive care is delivered regardless of circumstances.
• Report directly to the Medical Director and collaborate closely with operational leadership to meet clinical goals.

Patient Population / Client Focus

The Elderly Care & Rehab Clinic primarily caters to older adults on Medicare who often face complex health issues requiring personalized and attentive care. Our approach is centered on understanding the unique needs of geriatric patients, providing services tailored to maintain their health, dignity, and quality of life as they age.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Possession of a valid, active state Nurse Practitioner license without probationary status.
• Current DEA license to prescribe medications.
• Demonstrated experience in primary care settings, particularly in geriatrics, internal medicine, or family practice.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and health risk assessments.
• A collaborative mindset and a passion for teamwork that promotes quality patient outcomes.

Preferred Experience

• Prior experience working with older adult populations directly.
• Expertise in community health resource navigation.
• Proficiency in electronic health record (EHR) systems for efficient patient documentation and care coordination.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 per year, commensurate with experience and qualifications.
