Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, identifying both acute and chronic health issues.
• Diagnose and manage various medical conditions, implementing preventive care strategies and personalized health promotion plans.
• Work closely with school personnel and parents to deliver health education programs aimed at fostering healthy habits and awareness.
• Provide culturally competent care, advocating for patient rights and ensuring equal access to healthcare resources.
• Establish and maintain effective communication channels for referrals and follow-up care, guaranteeing continuity of service.
• Document all patient interactions and treatment provided in electronic health records (EHR), adhering to regulatory compliance standards.
• Engage in community outreach activities, health fairs, and educational workshops to promote health awareness among families in the community.
• Keep abreast of current community health trends, advancements in medical practices, and pertinent health policies.

Patient Population / Client Focus

Your primary focus will be on a diverse student population, including children and adolescents, as well as their families. You will address a range of health concerns reflective of the community's demographic, offering services that are sensitive to cultural variances and health disparities.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized certifying bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• An active and unrestricted Nurse Practitioner (APRN) license in New York State is required.
• Possession of a DEA and NPI number for prescription management is essential.
• Minimum of 2 years of clinical experience, preferably in a primary care, community health, or school-based setting.
• Exemplary clinical assessment and diagnostic skills.
• Strong interpersonal and communication abilities, facilitating effective collaboration with a range of stakeholders.
• Competency in utilizing electronic health record systems for patient management.

Preferred Experience

• Experience in school-based health settings or public health initiatives is highly desirable.
• Demonstrated ability to work independently while also being an active and collaborative team player.
