Key Responsibilities\n

• Support primary care providers by identifying, treating, and managing patients' mental health and psychosocial issues, while collaborating as an integral member of the healthcare team.

• Conduct detailed patient intakes, focusing on both diagnostic and functional evaluations, providing valuable recommendations for treatment goals and plans to primary care providers.

• Provide targeted, brief interventions to clients in need of mental health support, ensuring that care is appropriate to their current situations.

• Advise primary care providers on optimal referrals for patients to specialty mental health facilities when more intensive care is warranted.
\n
• Establish follow-up protocols to monitor patients' progress and adjust treatment plans based on their evolving needs.
\n
• Develop and implement relapse prevention strategies, assisting patients in achieving and maintaining stable mental health.
\n
• Identify patients at risk and collaborate on creating plans to prevent deterioration of their mental or physical health.
\n
• Document all patient interactions and treatment progress meticulously, ensuring compliance with agency policies and regulatory standards.
\n
• Provide patients with educational resources and self-management skills needed to actively participate in their recovery process.
\n
• Engage in ongoing consultation with clinic management and team members regarding behavioral services and program evaluation.

• Support patients in adhering to medical treatments prescribed by primary care providers, assisting with coping strategies for medication side effects.

Qualifications\n

Compensation and Benefits\n

The compensation for this remote LPC position ranges from USD 87,000 to USD 105,000 annually, commensurate with experience and qualifications.
