Join our homecare team and experience nursing the way it was meant to be personal, meaningful, and deeply rewarding.What you'll do as a resident!You will learn how to provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. You'll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference.Your responsibilities include:Deliver safe, patient-centered nursing care in the homeServe as the primary case manager, coordinating care with the interdisciplinary teamDevelop and implement individualized plans of careProvide patient and caregiver education, support, and disease management teachingEnsure accurate, timely documentation (documentation time built into the home visit)Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient's plan of careMaintain compliance with all infection control and regulatory standardsStay current with clinical best practicesWhat we're looking for!Valid driver's license, auto insurance, and reliable vehicleSelf-directed, organized, and comfortable working independentlyStrong communication and computer skillsCurrent New York state registered professional nurse license or may hold a graduate nurse license; BSN preferred (tuition reimbursement available!)Must attain RN licensure within 90 days of hire.Self-directed and organized with the ability to work independently.Basic computer knowledge with proficiency in Microsoft Office or other word processing, spreadsheet, and database management.Good oral and written communication skills.Demonstrates motivation to learn a new role and is an active learner.Physical requirements:Ability to sit or stand for long periods during work dayAbility to perform tasks involving physical activity (including but not limited to): pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.Ability to drive in all weather conditions (rain, sleet, snow, wind).Visual/hearing ability sufficient to comprehend written or verbal communicationAbility to use telephone and communicate over the phoneFrequently uses a laptop computer to enter orders and obtain product informationBloodborne pathogens exposure category I - employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.Compensation & benefitsCompetitive salary [this position is an S03 exempt position with a min-max rate of: $72,415.71- $83,992.40/annually] with:$15,000 sign-on bonusRetention bonusesInternal mentoring program401K with generous employer matchOn-site gym (free for all employees)Excellent work/life balance (no rotating shifts, only 2 required holidays per year on a rotating schedule)Medical, dental, vision plansTuition reimbursement (BSN after only 6 months of employment!)Partially funded HSAEmployee recognition platformPaid time off, holidays, sick and extended sick leaveShort/long term disabilityEmployee assistance program (EAP)Much more!About Nascentia Health:Nascentia Health is leading the way in home care, post acute care and long-term community health.
