As a therapist within our organization, you will report directly to the Clinical Manager and play a vital role in assisting patients to regain their strength, mobility, and independence—all from the comfort of their homes. By providing tailored care that emphasizes functional improvement, you will empower individuals to overcome challenges and enhance their quality of life.

Key Responsibilities

• Design and implement prescribed physical therapy treatments for patients in their homes, aiming for the highest level of physical function.
• Evaluate and document patients' physical capabilities to aid physician assessments and inform treatment objectives.
• Create and execute rehabilitation programs aligned with the physician's treatment plan and the goals of the patient care team.
• Provide clear written instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, supervising their performance and documentation.
• Thoroughly document patient observations, interventions, and evaluations in a timely manner.
• Communicate patient progress to the physician, Clinical Manager, care team, patient, and family, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.

Required Qualifications

• Degree from an accredited Physical Therapy Program recognized by the APTA.
• Current and valid Physical Therapy license.
• Current CPR certification.
• Experience working collaboratively with a healthcare team.

Preferred Qualifications

• At least one year of experience as a Physical Therapist.

Compensation

The pay range for this position is as follows:

• $59.00 - $83.00 per visit/unit
• Annual base salary between $93,000 and $128,000

Additional Information

• Tuberculosis Screening: This position involves direct patient interaction and is part of our TB screening program. Candidates selected for this role will need to undergo TB screening.
• Driving Requirements: A valid state driver's license is necessary, along with maintaining personal vehicle liability insurance. Candidates must meet the minimum insurance requirements of their state, or have coverage of at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.
• Weekly Hours: 40 hours per week.
