Work Environment

Key Responsibilities

• Perform initial assessments and ongoing evaluations of patient mobility, pain levels, strength, and functionality to identify specific needs and goals.
• Design and implement comprehensive individual treatment plans that align with therapeutic goals and enhance the patients' overall functionality.
• Deliver patient-centered, evidence-based therapy sessions, focusing on improving mobility, alleviating pain, and preventing subsequent injuries.
• Educate patients, families, and caregivers on therapeutic techniques, providing guidance on effective at-home exercises and wellness strategies.
• Work collaboratively with other healthcare professionals to develop and adjust interdisciplinary care strategies that cater to each patient's evolving needs.
• Maintain precise documentation consistent with Medicare and state regulatory requirements, ensuring all patient records are accurate and up-to-date.

Patient Population / Client Focus

Your primary focus will be on geriatric patients receiving home care services. The diverse backgrounds and varying health challenges of these individuals require a tailored approach to rehabilitation. You will work with patients recovering from surgery, managing chronic conditions, or overcoming functional limitations due to age-related issues, ensuring that each resident's path to recovery is addressed with respect and thoroughness.

Qualifications

• A current and valid Physical Therapist license in Virginia or eligibility for licensure.
• A Bachelor's or Doctorate degree in Physical Therapy from an accredited institution.
• CPR/BLS certification, or willingness to obtain it upon hire.
• Demonstrated strong communication, empathy, and organizational skills essential for patient interaction.
• Ability to work both independently and as part of a cohesive, collaborative care team committed to patient rehabilitation.

Preferred Experience

• Two years of experience in physical therapy, particularly within geriatric or home care settings, is preferred but not mandatory.
• Previous experience with rehabilitation techniques and modalities specific to older adults.

Compensation and Benefits
