Job Title: Resource Access Coordinator (long-term, grant funded 1099 Contractor)
Immediate Supervisor : Community Resource Manager
Classification: 1099 Contractor (This is a long-term, 3-year/40 hours a week 1099 Contractor opportunity with one of the St. Louis Area Foodbank's premier partners - SSM Healthcare. Depending on organizational business needs and budgetary considerations, there is a possibility that this role may be converted to a full-time position with the Foodbank after the conclusion of the 3-year assignment as a 1099 Contractor.)
General Description/Position Summary:
In July 2023, the St. Louis Area Foodbank expanded our partnership with SSM Healthcare to address both food security and access to medicine. This program grew from one hospital in the St. Louis area to seven, highlighting our increased need to enhance our capacity. This position will collaborate closely with the Community Resource Manager and the Community Healthcare Programs Coordinator to connect our neighbors with essential resources. This role will require a commitment of 40 hours per week.
Duties/Responsibilities:
Public Benefit Application Assistance:
- Conduct pre-screenings and complete Public Benefit Applications for all UniteUs referrals.
- Ensure accurate data collection, maintain tracking tools, and compile reports effectively.
Referral Management:
- Oversee all healthcare referrals generated by the SSM Healthcare system within the UniteUs platform.
- Monitor referred patients using the St. Louis Area Foodbank tracking tools.
- Serve as the primary contact for referrals outside of our SSM Health contract on the UniteUs platform.
Survey Administration:
- Conduct follow-up surveys via phone to gather information on patients who utilized resources post-discharge from hospital.
- Conduct satisfaction Surveys for all applications completed through UniteUs platform.
Food Security Trends Tracking:
- Assess and track food security trends in patients at one- and three-month post-hospital discharge using questions derived from the USDA's Short Form Food Security Survey Model.
Data Collection:
- Record and analyze survey data, collaborating with the Community Resource Manager and Community Healthcare Programs Coordinator.
Collaboration:
- Work with Community Resource and Programs teams to enhance understanding of patient needs and challenges.
- Other duties as assigned.
Qualifications:
- Bachelor's degree from an accredited university or college is required, a degree in public health, nutrition, or social work is preferred.
- 1-3 years previous experience coordinating programs and/or volunteer activities with partners and volunteers of varying socio-economic backgrounds for a non-profit organization strongly preferred.

- Experience working with state, federal or grant funding preferred.
- Must have reliable transportation with proof of insurance and be willing to travel within a 125-mile radius if needed.
- Must be able to pass drug screening and criminal background check.
Work Schedule:
- 40 hours a week.
- Special events and other projects related to this position may require occasional weekend and/or evening hours and work.

- The St. Louis Area Foodbank (SLAFB) is a member of Feeding America, which is a national network of 200 food banks all working together for the common goal of ensuring equitable access to nutritious food

- As a food bank, we collect food from a variety of sources, such as manufacturers, retail stores, farmers, and individual donors
- We then store, sort, and repackage that food at the highest safety standards and finally distribute that food to our network of agencies and programs, which work directly with people in the community
- We facilitate the movement of food safely and efficiently and offer the kinds of resources and support to our partner agencies that a single food pantry or shelter could not hope to access on its own
