Key Responsibilities

• Conduct thorough evaluations that include reviewing patient histories, performing specific assessments, and formulating documentation that outlines treatment recommendations.
• Explain evaluation findings and collaborate with patients and their families to establish achievable goals.
• Demonstrate proficient execution of treatment protocols tailored to patient evaluations and consistently adapt these protocols based on each patient's progress.
• Consult and collaborate with other healthcare providers to optimize patient care and address any emerging needs.
• Maintain meticulous records of patients' progress through accurate documentation, including initial evaluations, daily encounter notes, interim summaries, and discharge plans.
• Manage your schedule effectively, ensuring timely attendance at meetings and participation in team initiatives aimed at enhancing patient access and experiences.
• Engage in professional development opportunities, including serving as a clinical instructor to students pursuing their degrees in physical therapy.
• Actively promote patient-centered care by eliminating barriers to access and facilitating seamless transitions throughout the rehabilitation journey.

Patient Population / Client Focus

As a Full-Time Physical Therapist at Swift Health, you will work with a diverse patient population, ranging from individuals recovering from surgical procedures to those managing chronic pain or musculoskeletal disorders. Qualifications

• A Bachelor's Degree in Physical Therapy is required; however, a Doctorate degree in Physical Therapy (DPT) is preferred for enhanced clinical knowledge.
• A valid state license for physical therapy is necessary to practice in New Jersey.
• Current American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers is required.

Compensation and Benefits

The annual salary for this role ranges from $100,000 to $120,000, reflecting your skills and experience.
