Key Responsibilities\n

• Conduct thorough evaluations and assessments to understand the needs of children enrolled in our hospice programs.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring an evidence-based approach tailored to each child's unique situation.

• Monitor progress and outcomes, collaborating with team members to foster a supportive and effective care environment.
\n
• Provide families with vital resources, educational materials, and emotional support to enhance their child's development and coping strategies.

• Engage in continuous compliance with State and Federal regulations concerning child welfare and early intervention practices.

• Develop strong rapport with children, families, and team members, promoting trust and a healthy learning environment.

• Utilize coaching models and natural environment teaching techniques to deliver interventions effectively.
\n
• Implement strategies based on the family's priorities, recommending service frequency aligned with each child's developmental milestones.

Patient Population / Client Focus\n

In this role, you will primarily work with young children facing disabilities and developmental delays, as well as their families. Understanding the specific challenges these families encounter is crucial. You will be a key member of the care team, collaborating with families to provide personalized support that encourages the child's growth in physical, cognitive, social, emotional, and communication skills during their critical developmental years.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Valid LCSW (Licensed Clinical Social Worker) credential issued by the appropriate regulatory agency.

• Current licensure by the Department of Public Health.

• Must possess a valid driver’s license.

Compensation and Benefits\n
