Physical Therapist

Job Details
Evaluate residents' physical abilities, mobility limitations, and rehabilitation needs.
Develop and implement individualized treatment plans based on physician orders and patient goals.
Provide therapeutic exercises, gait training, balance training, and mobility interventions.
Monitor patient progress and adjust treatment plans as needed to maximize outcomes.
Educate residents, families, and caregivers on exercises, safety techniques, and injury prevention.
Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate care.
Maintain accurate and timely documentation of evaluations, treatments, progress notes, and discharge summaries.
Ensure compliance with facility policies, regulatory requirements, and professional standards of practice.
