Key Responsibilities:

- Perform comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and IV therapies as prescribed
- Monitor residents' conditions and report changes to physicians and care teams
- Maintain accurate and timely clinical documentation in accordance with state and federal regulations
- Ensure compliance with infection control, safety, and quality standards
- Communicate effectively with residents, families, and interdisciplinary team members
- Provide patient and family education on health conditions and care needs
