Trigger Physical Therapy is seeking a dedicated and experienced Physical Therapist to enhance the lives of our clients through innovative home care solutions. In this full-time role based in Passaic, New Jersey, you will apply your expertise in movement science to assess, manage, and optimize physical rehabilitation outcomes for patients in a comfortable home environment.

Work Environment

This position is situated within a home care setting, allowing you to meet clients in their personal spaces, which can enhance comfort and engagement during therapy sessions. You'll work from Monday to Friday, 9 AM to 5 PM, allowing for a balanced work-life integration while focusing on delivering high-quality physical therapy in a personalized context.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical capabilities and needs, developing tailored treatment plans that address varying levels of mobility and rehabilitation goals.
• Implement and manage therapy sessions for one patient per hour, skillfully offering individual attention and care to optimize treatment effectiveness.
• Utilize advanced manual therapy techniques and modern rehabilitation methods to ensure efficient and effective patient outcomes.
• Monitor and document patient progress meticulously, maintaining up-to-date records in compliance with industry standards and regulatory requirements.
• Collaborate with multidisciplinary healthcare teams to align on best practices for patient care and ensure holistic treatment approaches.
• Engage with patients and families to educate them on techniques and exercises that can enhance mobility and independence beyond therapy sessions.

Patient Population / Client Focus

Your caseload will vary, allowing you to work with a diverse range of patients—ranging from individuals recovering from orthopedic surgeries to those rehabilitating from chronic conditions. This position requires a compassionate approach to care and an understanding that each patient's journey is unique.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid New Jersey license to practice as a Physical Therapist.
• Minimum of two years of hands-on experience in providing physical therapy services in a relevant healthcare setting.
• A clean license with no disciplinary actions or legal infractions.
• Expertise in manual therapy techniques to facilitate patient rehabilitation.
• Capacity to work autonomously while also thriving in collaborative, team-oriented settings.

Preferred Experience

Experience working in a home care environment is highly desirable, as is familiarity with documentation practices specific to home health therapy. Proficiency in navigating electronic health records (EHR) systems will be advantageous for maintaining accurate patient information.

Compensation and Benefits
