Transformative Rehab & Physical Therapy is seeking a motivated and dedicated Physical Therapist to join our dynamic team. This full-time position, located in Hoboken, New Jersey, involves working closely with patients in a home care setting, focusing on maximizing their quality of life and movement potential. Work Environment

You will be working onsite within clients' homes, allowing you to build close, trusting relationships with patients while providing personalized care. Our office hours are Monday to Friday, 9 AM to 5 PM, offering a consistent schedule that promotes work-life balance. As part of a collaborative and supportive team, you will have access to necessary tools and resources to ensure effective treatment and outcomes for your patients.

Key Responsibilities

• Conduct thorough patient evaluations to identify individual needs and establish personalized treatment plans in collaboration with the patient and their families.
• Deliver one-on-one therapeutic services, managing a typical patient load of one patient per hour, tailoring approaches based on the patient's progress and ongoing needs.
• Utilize advanced manual therapy techniques to address musculoskeletal conditions and improve patients' mobility.
• Document all patient interactions meticulously, including evaluations, treatment plans, progress notes, and billing information, in compliance with healthcare regulations.
• Monitor patient progress closely, adjusting treatment strategies as necessary, and maintaining open lines of communication between the patient and the healthcare team.
• Engage in interdisciplinary collaboration with other healthcare professionals to ensure a comprehensive approach to patient care.

Patient Population / Client Focus

Our patient population includes individuals recovering from surgery, experiencing chronic pain, or managing disabilities that affect their mobility. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid license to practice as a Physical Therapist in New Jersey.
• At least two years of professional experience providing physical therapy services.
• A clean record with no disciplinary actions or board sanctions against your license.
• Proven expertise in manual therapy techniques and effective treatment modalities.
• Strong interpersonal skills, with the ability to work both independently and collaboratively within a healthcare team.

Preferred Experience

Experience in orthopedic outpatient environments is particularly valued, as is familiarity with various therapeutic modalities and patient-focused care strategies. The ability to adapt treatment approaches based on patient feedback is essential for successful outcomes.

Compensation and Benefits
