Work Environment

This position is primarily onsite, where you will visit patients in their homes, providing personalized care in a comfortable and familiar setting. Our home care facility allows therapists to deliver hands-on treatment tailored to individual patient needs. Key Responsibilities

• Conduct thorough assessments to evaluate patients' physical condition, including mobility, strength, and overall functional abilities.
• Create and implement customized treatment plans utilizing advanced manual therapy techniques, therapeutic exercises, and neuromuscular re-education.
• Monitor and document patient progress meticulously, adjusting treatment strategies based on real-time feedback and clinical outcomes to enhance recovery.
• Educate patients and their families on home exercise programs, injury prevention strategies, and wellness techniques, empowering them with the knowledge to maintain their health.
• Collaborate with physical therapy assistants, physicians, and other healthcare professionals to ensure integrated care and achieve the best patient outcomes.
• Proactively communicate any changes in a patient's condition to relevant stakeholders, including the supervising physician and family.

Patient Population / Client Focus

Your primary focus will be on patients with orthopedic conditions, including those recovering from surgeries, managing chronic pain, and sports injuries. You will work closely with various demographics, including adults and seniors, requiring an understanding of and sensitivity to the unique needs of each age group.

Qualifications

• An active Physical Therapist license in New Jersey is required.
• A Bachelor's degree in Physical Therapy is the minimum qualification, though a Doctorate or Master's degree is preferred.
• At least two years of professional experience in physical therapy, with a focus on outpatient orthopedics preferred.
• Demonstrated ability to communicate effectively with patients, families, and team members, showcasing strong clinical knowledge and empathy.
• Current CPR and First Aid certification is preferred; additional credentials in geriatrics or neurological rehabilitation are a plus.
• A valid driver's license and reliable transportation are mandatory to facilitate home visits.

Preferred Experience

• Experience specifically within home care settings is advantageous.
• Interest or certification in specialized therapeutic techniques related to orthopedic rehabilitation is desirable.

Compensation and Benefits

The role offers a competitive salary range of $90,000 – $100,000 annually, commensurate with experience and qualifications.
