Work Environment

As a Physical Therapist within our home care setting, you will engage directly with patients in their homes, fostering a comfortable and familiar environment for therapy. Key Responsibilities

• Administer comprehensive evaluations of patients' physical abilities to customize effective rehabilitation plans.
• Utilize manual therapy techniques and evidence-based practices to alleviate pain and enhance mobility.
• Design and implement targeted therapeutic exercise programs specific to each patient's rehabilitation goals and lifestyle needs.
• Coordinate with a multidisciplinary team, including physicians and nurses, to ensure seamless patient care and adherence to treatment protocols.
• Educate patients and families regarding rehabilitation processes, in-home exercises, and preventative care strategies to promote sustained health benefits.
• Maintain accurate and compliant documentation of patient assessments, progress notes, and treatment outcomes in alignment with regulatory standards.

Patient Population / Client Focus

Our physical therapy department serves a diverse patient population primarily focused on orthopedic conditions. Qualifications
