Work Environment

This is an onsite position where therapists will provide services directly within the patient's home environment, allowing for personalized care that addresses specific individual needs. Key Responsibilities

• Conduct comprehensive evaluations of new patients, identifying physical conditions and functional limitations through detailed assessments.
• Create individualized treatment plans that reflect the unique needs and goals of each patient, utilizing evidence-based practices and advanced therapeutic techniques.
• Implement a range of therapeutic interventions, including manual therapy, strength training, and modality application, to enhance recovery and functionality.
• Provide education to patients on injury prevention, physical activity modifications, and safe exercise practices to maximize recovery potential.
• Documentation of patient progress in real-time and modification of treatment plans based on responsiveness to therapy.
• Collaborate with physicians, occupational therapists, and other healthcare providers to facilitate comprehensive patient care and ensure quality treatment outcomes.
• Participate in team meetings and case discussions to share insights and strategies that optimize patient care.

Patient Population / Client Focus

Your therapeutic expertise will primarily benefit patients experiencing a variety of conditions, such as chronic pain syndromes, joint replacements, post-surgical rehabilitation, sports injuries, balance disorders, and other mobility issues. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current state licensure to practice as a Physical Therapist in New Jersey.

• Proficiency in various physical therapy techniques and modalities relevant to home care settings.

Preferred Experience

Compensation and Benefits
