Work Environment

This full-time position is an onsite role within the home care segment of our organization. You will provide therapy in patients' homes, adapting your approach to each unique setting and individual. Our team operates Monday through Friday, from 9 AM to 5 PM, allowing you to maintain work-life balance while delivering impactful care. The environment is collaborative, where physical therapists engage with a multidisciplinary team to ensure comprehensive treatment plans are executed effectively.

Key Responsibilities

• Conduct thorough assessments of patients' physical abilities, performing specialized evaluations to identify their challenges and strengths.
• Develop and implement tailored treatment programs that align with each patient's unique recovery goals, utilizing techniques such as manual therapy and therapeutic exercises.
• Engage patients in active rehabilitation, ensuring they understand each component of their treatment plan, which promotes adherence and motivates progress.
• Monitor patient progress meticulously, making adjustments to therapy plans as needed based on ongoing evaluations and patient feedback.
• Collaborate extensively with physicians, occupational therapists, and other healthcare professionals to ensure an integrated approach to patient care and improving outcomes.
• Document patient interactions and treatment plans accurately using electronic medical record systems, such as WebPT, to keep all stakeholders informed and up-to-date.

Patient Population / Client Focus

Your primary focus will include patients dealing with a range of conditions, including musculoskeletal injuries, neurological disorders, and cardiovascular issues. You will assess each individual's specific needs, implement recovery solutions, and track evolution throughout the rehabilitation process—all while fostering a supportive environment in their own home where they feel comfortable and encouraged.

Qualifications

• A Master's or Doctorate in Physical Therapy from an accredited institution.
• Current and active license to practice Physical Therapy in New Jersey.
• A minimum of 2 years of clinical experience in a similar setting, with specific expertise in outpatient therapy.
• Demonstrated proficiency in manual therapy techniques and familiarity with rehabilitation equipment.

Preferred Experience

• Experience using EMR software, particularly WebPT, to enhance patient documentation and care coordination.
• Previous involvement in home care settings, demonstrating the ability to adapt treatment plans effectively in diverse environments.
• Strong communication skills for effective patient interaction and collaboration with interdisciplinary teams.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
